NOTICE
Must be 18 or older and have a valid ID to be tattooed
No one under the age of 16 is allowed in the shop.
ABOUT REQUESTS
This form will allow me to get the necessary information to design your tattoo with best of my ability and save a lot of back and forth questions.
If requesting multiple tattoos, or for more than one person please specify on form under the design description.
Appointment requests will be responded to within 1 week from lilylaurilatattoos@gmail.com
-Please check your spam folder if you do no see a response in that time
Reasons why requests may not be accepted:
- I am booked for the current quarter
(I reopen my books every 3 months)
-It does not fall under my tattoo style
(other artist recommendations are often made in this case)
A $50 deposit may be required for medium- large scale tattoo designs.
